Overview

Forms are in PDF format. You must have at least Adobe Acrobat Reader 4.0 or a later version to view these forms.

Download the latest version of Adobe Acrobat Reader. These forms are no longer available at the Bursar's Office.

Forms can be completed, signed and submitted to the Bursar's Office by:

  • Email: bursar@sfsu.edu
  • In person -Student Service Building
  • Fax to (415) 338-7787
  • Mail to the address below, along with any required payments and/or additional documentation.
  • Refund requests can be submitted online through your Student Center, under the “Resources” section located at the bottom right corner of the page.

Official Receipt/Invoice Request

To request an official receipt or invoice, please send an email to bursar@sfsu.edu.

  • Please allow up to 20 business days for processing.
  • Prior year requests will incur a $20 fee per academic year.

Refund Request for Tuition and Fees: Please submit your request through your Student Center by selecting “Tuition and Fees Refund” under the Resources section. If you experience any issues or have questions, please email your request to bursar@sfsu.edu from your SFSU email address.
